Abstract
Telehealth has transformed healthcare delivery by expanding clinical services across geographical and institutional boundaries using telecommunications and digital infrastructure. Applications span teleconsultation, remote patient monitoring (RPM), telepsychiatry, teledermatology, telepharmacy, digital therapeutics, and virtual multidisciplinary care. While operational before the COVID-19 pandemic, crisis conditions substantially accelerated adoption and demonstrated telehealth’s capacity to maintain care continuity. Evidence confirms that telehealth broadens medical access—particularly for rural and remote populations—reduces travel times, streamlines chronic disease management, and enhances patient convenience.
However, implementation faces substantial operational barriers, including infrastructure deficits, digital inequity, privacy and cybersecurity vulnerabilities, regulatory uncertainties, clinician burden, and the inability to conduct physical examinations. Because telehealth is not uniformly effective across all clinical conditions, the optimal delivery framework relies on integrated hybrid care models that combine virtual and in-person consultations. In India, the 2020 Telemedicine Practice Guidelines provided a formal regulatory base, though infrastructure, training, and sociocultural barriers persist. Future progress will depend on artificial intelligence, wearable biosensors, interoperable electronic health records, and personalized clinical decision support. Sustainable expansion requires evidence-based regulation, equitable funding, robust security, and targeted strategies to prevent digital exclusion.
