Readiness Determination in School-Based Trauma Intervention: How Clinicians Decide When a Child Can Safely Engage Trauma-Focused Treatment

Abstract
Child trauma treatment endorses safety and stabilization before trauma processing, yet determining when a child is ready remains inadequately operationalized in school-based mental health care across the United States. Premature progression can precipitate symptom escalation, dissociation, shutdown, disengagement, or treatment dropout, while school schedules, group delivery, limited caregiver involvement, environmental instability, and variable trauma-specific preparation complicate clinical judgment. This paper examines readiness provisions across CBITS, Bounce Back, SSET, TF-CBT, SPARCS, ARC, TARGET, and EMDR, coding whether readiness criteria are explicit, implied, absent, or dependent upon clinician judgment. It distinguishes symptom measurement from readiness determination and identifies a related supervisory gap: verifying clinician capacity to make and document this decision. The paper introduces Creating Conditions for Becoming, a threshold-gated framework assessing affect regulation, environmental stability, supportive-adult availability, therapeutic alliance and willingness, dissociation, developmental capacity, and observable readiness loss. Progression requires documented supervisory verification rather than predetermined session counts. The framework provides a testable structure for clinical practice, supervision, and training, while establishing priorities for Delphi consensus, readiness-instrument validation, and school-based outcome research supporting safer trauma treatment nationally.
