Abstract
Psoriasis is a chronic, immune-mediated inflammatory dermatosis characterized by recurrent erythematous, well-demarcated plaques with scale and variable scalp, nail and musculoskeletal involvement. Modern models describe an interaction between genetic susceptibility, environmental triggers, keratinocytes and innate/adaptive immunity, with the IL-23/IL-17 axis having a central role. In classical homoeopathy, management is based on individualization and the totality of symptoms, with particular attention to peculiar symptoms, modalities, concomitants and constitutional features. This critical review examines the classical miasmatic interpretation of chronic psoriasis and differentiates selected remedies described in standard materia medica, including Sulphur, Graphites, Arsenicum album, Petroleum, Natrum muriaticum, Mercurius solubilis, Calcarea carbonica and Lycopodium. The review also considers published clinical evidence without equating homoeopathic theory with biomedical pathophysiology. A 2009 prospective observational study of 82 adults reported improvement during individualized homoeopathic care, but its uncontrolled design limits causal inference. A 2023 double-blind randomized placebo-controlled trial of 51 patients reported greater PASI improvement at six months in the individualized-medicine group, while total PDI and total DLQI differences were not statistically significant. The evidence remains limited and does not justify replacing established dermatological treatment. Miasmatic concepts are therefore presented as a historical homoeopathic framework for case analysis rather than as validated biological mechanisms.
