Abstract

Acne vulgaris is a widespread, multifactorial, chronic inflammatory skin disorder of the pilosebaceous unit, characterized by excessive sebum production, follicular hyperkeratinisation, microbial colonization by Cutibacterium acnes, and subsequent inflammation that exerts a profound psychosocial and clinical burden on affected populations across varying severity grades. While conventional topical and systemic therapies remain the mainstay of treatment, their long-term clinical utility is often limited by cutaneous irritation, systemic adverse effects, and escalating antimicrobial resistance. In response to these clinical challenges, polyherbal topical formulations have emerged as a promising alternative, imparting synergistic phytoconstituents to exert multi-target therapeutic effects. In response, phytomedicine has emerged as a promising alternative, leveraging protective synergy wherein diverse bioactive botanical constituents act simultaneously on multiple pathogenic pathways with reduced adverse profiles. This review comprehensively examines the anatomical and pathophysiological mechanisms underlying acne. It evaluates clinical management strategies by contrasting conventional versus advanced, and topical versus systemic interventions. Furthermore, the work details the formulation design, rheological parameters, basic gel components, and evaluation protocols (including organoleptic, physicochemical, and preliminary stability parameters) for polyherbal gel delivery systems. By synthesizing multi-herbal strategies, this review establishes a structured rationale for developing stable, patient-compliance, and effective polyherbal topical gels aimed at Curing inflammatory acne while minimizing adverse effects.
