Abstract
Adolescent substance abuse and co-occurring mental health disorders present a complex, multifaceted public health crisis. Traditional treatment paradigms historically prioritized the individual youth as the primary unit of intervention. However, contemporary clinical research indicates that adolescent behavioral health is deeply embedded within the dynamic family system. This descriptive research paper comprehensively evaluates the efficacy, operational mechanisms, and systemic outcomes of evidence-based family-focused interventions (FFIs)—specifically Multidimensional Family Therapy (MDFT), Functional Family Therapy (FFT), Multisystemic Therapy (MST), and Brief Strategic Family Therapy (BSFT).
The primary objectives of this study are to systematically describe these models, identify the relational mechanisms driving behavioral modification, examine empirical outcomes regarding substance reduction and psychological stabilization, and critically assess implementation barriers. Synthesized findings demonstrate that family-based treatments yield superior or equivalent long-term clinical gains compared to individual counseling and residential care, primarily by restructuring maladaptive systemic dynamics like poor parental monitoring, inconsistent discipline, and high family conflict [Austin, 2005; Hogue, 2018]. However, widespread adoption remains limited by clinical attrition, intensive training costs, and logistical burdens.
